INTRODUCTION TO BUDDHISM COURSE
REGISTRATION FORM
*** WE ARE CURRENTLY HAVING
SOME PROBLEMS WITH OUR EMAIL ACCOUNT - PLEASE CONTINUE TO FILL OUT THE
FOLLOWING FORM, BUT IF YOU HAVEN'T HEARD FROM US IN 3 OR 4 DAYS, PLEASE
DO EMAIL US AT spc(at)tushita.info TO CHECK THAT WE RECEIVED YOUR FORM.
APOLOGIES (we're working on it!) AND THANKS FOR YOUR PATIENCE***
Before
filling out this registration form please make sure
that you carefully read
all the information on our website regarding the Introduction to Buddhism courses
(basic
course discipline, what
to bring etc.) Please register
for a course only if you are sure
you
will attend.
Please make also sure that you are in sound
physical & mental
health when you come to attend one of our courses! If you feel sick
please try to visit a doctor to receive adequate advice and medications
BEFORE you start the course.
- If you have diarrhoea
we strongly advise you to get a stool
test done, so you are confident about what is causing your
sickness and that you take the right medications!
- If you are experiencing flu-like symptoms, please
seek medical advice before
joining the course. We must consider the welfare of all students on our
group retreats.
- Our experience at Tushita has shown that
meditating while taking Larium and other malaria prophylaxis may
increase the likelihood and/or severity of the medicament's side
effects and therefore we
discourage students from taking Larium while meditating or
participating in our courses (there is no malaria at
Tushita).
For more information please visit our health
page.
Once
you have registered for the course and have received the
registration confirmation email, there is nothing else you must do.
Cancellations:
Please keep in mind that most of our introductory courses have long
waiting lists by the time the course starts. If you later realize that
you cannot attend the course, PLEASE
let us know (just send a short
email), so as not to deny anyone else the chance to attend.
This
form will be read only by the course
teachers and Tushita management.
Any information given will
be held
in strict confidence.
MAY ALL BEINGS BE HAPPY!
